If you are applying for permanent residence through Express Entry, completing an Immigration Medical Exam (IME) is no longer optional or something you can wait for IRCC to request later.
As of August 21, 2025, an upfront Immigration Medical Exam (IME) is mandatory for all Express Entry permanent residence applications. Applications submitted without a valid upfront IME are considered incomplete and may be refused.

What Is an Immigration Medical Exam (IME)?
An Immigration Medical Exam (IME) is a health examination required under Canadian immigration law to determine whether an applicant:
- Is not a danger to public health
- Is not a danger to public safety
- Is not reasonably expected to cause excessive demand on Canada’s health or social services
(unless exempt under Canadian law)
IRCC cannot approve a permanent residence application unless a valid IME exists.
What Does “Upfront IME” Mean in Express Entry?
An upfront IME means:
- You complete the Immigration Medical Exam before submitting your Express Entry PR application
- You do not wait for IRCC to issue a medical request
Critical Rule (Effective August 21, 2025)
For all Express Entry applications submitted on or after August 21, 2025:
- IRCC will not request medical exams later
- A PR application submitted without an upfront IME is incomplete
- Incomplete applications may be refused
Who Is Required to Complete an IME?
Principal Applicant
Every principal applicant applying for permanent residence through Express Entry must have a valid IME.
Family Members (Accompanying and Non-Accompanying)
All family members must also complete an IME, even if they are not accompanying the principal applicant:
- Spouse or common-law partner
- Dependent children
Non-accompanying family members are not exempt from medical examination requirements.
Temporary IME Exemption for Applicants Inside Canada (Valid Until October 5, 2029)
IRCC has extended a temporary public policy that allows certain applicants inside Canada to be exempt from completing a new IME.
This exemption is valid until October 5, 2029.
When the IME Exemption Applies
You may not need to complete a new IME if:
- You are applying from inside Canada
- You completed a previous IME for a Canadian immigration application
- You include the IME number or unique medical identifier from that previous exam
Conditions You Must Meet to Be Exempt (All Required)
You may reuse a previous IME only if all of the following are true:
- You applied or are applying for temporary or permanent residence
- You currently live in Canada
- Your previous IME was completed within the last 5 years
- The medical results showed low risk or no risk to public health or public safety
If any one condition is not met, a new IME is required.
How the 5-Year IME Window Works (Example)
You may reuse a previous IME if it was completed for:
- A work permit
- A study permit
- Any other Canadian immigration application
Example Scenario
- You completed an IME for a work permit
- You are now applying for permanent residence through Express Entry
- The IME completion date and your PR submission date are within 5 years
If your medical results indicated low or no risk, you may be exempt from completing a new IME.

How to Complete an Upfront Immigration Medical Exam
To complete an upfront IME correctly:
- Find an IRCC-approved panel physician
- Book an Immigration Medical Exam
- Attend the exam and complete required tests
- The physician submits results electronically through eMedical
- You receive a medical information sheet
- Upload the information sheet with your PR application
Important
- Applicants do not upload medical reports or test results
- Only the medical information sheet is uploaded
Who Can Perform an Immigration Medical Exam?
- Only IRCC-authorized panel physicians can perform IMEs
- Regular family doctors cannot perform immigration medical exams
Limited Exception (Inside Canada Only)
In rare cases, IRCC may authorize a non-panel physician when access to a panel physician is unreasonable due to:
- Language barriers
- Severe health limitations
- Geographic isolation
This requires prior IRCC approval and results in slower, paper-based processing.
Non-Accompanying Family Members: Important IME Rules
- Non-accompanying family members must complete one IME
- If the IME later expires, IRCC may request a medical reassessment
- A full repeat IME is not normally required
If a Required Family Member Does Not Complete an IME
- The PR application may be refused
- IRCC cannot assess medical admissibility
- This is considered a serious non-compliance issue
Humanitarian and Compassionate (H&C) IME Exceptions
In exceptional circumstances only, IRCC may waive the IME requirement where:
- A separated spouse’s whereabouts are unknown
- A family member refuses to undergo an IME
- A parent refuses consent for a child’s medical exam
If an exemption is granted:
- The family member must be listed as non-accompanying
- The principal applicant must acknowledge that they may be barred from sponsoring that family member in the future
